Infants & Children
While Coastline DPC is eager to serve entire families, at this point in time we are unable to care for children 0-5 years of age. Children over the age of 6 will need to receive all childhood immunizations from the pharmacy (patient/insurance to pay).
Enrollment
- One time enrollment fee of $100 per person or $175 enrollment fee per family.
- There are no fees associated with pre-enrollment.
- A re-enrollment fee of $150 is charged for lapsed memberships.

Direct primary care is an incredible benefit to offer employees.
